You’ve already taken the first step by seeking us out!

Next, fill out the intake form found in the menu bar above under “Forms”. Once it is completed in its entirety, the form will be sent automatically to Play to Thrive.

Once we receive the form, we will reach out to schedule an initial evaluation for your child.

After the initial evaluation appointment is completed, your therapist will complete a written report with recommendations explaining how best to proceed.

If warranted, we will schedule a reoccurring, weekly appointment for your child to come in and participate in therapy!

At our practice we go above and beyond to keep our children safe. This form acknowledges that we do use gym equipment in an attempt to make therapy fun and interactive.
The Notice of Privacy Practices clearly states the responsibilities of Play OnWords, the uses and disclosures of protected health information as well as the patient's rights to privacy. The Acknowledgement of Receiving the HIPAA Form is a form for the client to sign that acknowledges that they have seen the HIPAA notice and understand its contents.
This form gives permission to the private practitioner to complete an evaluation and render appropriate therapy services.
